Interesting question that sometimes comes up, doesn't it? I remember feeling surgically inclined and forward-looking entirely at the time. 27 years on, I couldn't say there would have been anything I "wish I had done" with the foreskin at the time, while I still could. It was a well-known item with a history of years of not really wanting to have it, and I had done everything with it I could imagine to do by then.
One thing I would do though, also at the risk of pissing off the surgeon, is to go through in detail how you want this done again, perhaps using a pen. This is about getting your personal vision implemented now - don't miss the opportunity. You'll want to communicate everything. How far back exactly will it be cut, at what angle is the cut, is there a v-shape or not, sutures or glue, how tight. Leave nothing to chance if you have a vision.
